Special Request (Click for instructions)

Students submit this form when making out-of-the-ordinary academic requests of the College of Liberal Arts. Instructions:

  • List your name, student ID, contact information, major and minor, degree, and GPA.
  • State your request specifically, and explain why you are making the request.
  • Include the name and contact information of the advisor with whom you have discussed your request.
  • Sign, date, and submit the form to the Liberal Arts Advising Center, FH 322, for processing.
  • Average processing is 1-2 weeks.

Request to Take Courses Off-Campus (Click for instructions)

This form is submitted by students wishing to complete courses away from Texas State during their last semester of coursework. Instructions:

  • Include student name and ID, contact information, major and minor, degree, and GPA.
  • State which course will be taken away from Texas State, and provide the Texas State course that will serve as the course equivalent.
  • Sign, date, and submit the form to the Liberal Arts Advising Center, FH 322, for processing.
  • Average processing is 1-2 weeks.

Request for "If Letter" or "Letter of Completion" (Click for instructions)

With this form, students can request a letter stating their intention to complete a degree and the anticipated or actual completion date. The form is used typically by students who have received job offers and graduate school admission before completing an undergraduate degree at Texas State. Instructions:

  • List your name and student ID, major and minor, degree, and social security number if desired.
  • If the request is for an "If Letter," check the appropriate box and state the anticipated graduation date if current coursework is incomplete.
  • If all coursework is complete, check "Letter of Completion" and list the date of graduation.
  • List a phone number or email address where you can be contacted when the letter is ready for pick-up.
  • Sign, date, and submit the form to Liberal Arts Advising Center, FH 322, for processing.
  • Average processing is 1-2 weeks.

Petition for an Administrative "W" (Click for instructions)

Use this form to request that a posted grade be changed to "W" due to extenuating circumstances. 

Instructions:

  • List name, student ID, address, phone number, email, and major and minor.
  • List the course name, number, section, department, instructor, semester, and year the course was taken.
  • Attach a signed and dated personal letter, addressed to The Review Committee, explaining the circumstances that justify the request for a grade change. 
  • Submit the form and the letter to Liberal Arts, FH 313, for processing.
  • Average processing is 1-2 weeks.

Academic Suspension Appeal (Click for instructions)

If you wish to appeal an academic suspension, call the Liberal Arts Advising Center immediately at (512) 245-1852, to schedule an appointment with your advisor.

Transfer Course Evaluation Request (Click for instructions)

This form is used by transfer students who are transferring core coursework without direct equivalents at Texas State.  Use this form to have these courses evaluated.

Instructions:

  • Complete the form below, listing all courses completed in the core curriculum through another institution.
  • Include a copy of the Texas State evaluated transcript.  This should show both the original number of the course and what it was evaluated as.
  • Include as much of the following as possible:
    • a copy of the courses's catalog description;
    • a copy of the Table of Contents pages from the textbook used in the course;
    • the course syllabi and/or course materials;
    • correspondence from the instructor explaining contents of the course.
  • Submit these materials to Dr. Nelson in University College, ASBN 100 for processing.
